The Clinical Service Model

HarmonyBridge delivers eight integrated services spanning the full journey from hospital discharge to healthy living at home. Together they create continuity of care that no single-service provider offers, a child moves seamlessly from transitional medical care, through multidisciplinary rehabilitation and specialist clinics, into home healthcare with the family trained to continue care. The profiles below set out each service and its role.

SERVICE 1 Transitional Medical Care

24-hour step-down nursing

The clinical core: children discharged from public and private hospitals, neonatal ICUs, trauma, oncology and neurosurgery units receive 24-hour specialised nursing, medication management, nutrition, respiratory support and wound care. This step-down care safely frees acute hospital beds while giving fragile children the sustained clinical attention they need to stabilise and recover.

SERVICE 2 Comprehensive Rehabilitation Institute

Multidisciplinary therapy

One integrated rehabilitation centre combining physiotherapy, occupational therapy, speech and feeding therapy, neurodevelopmental therapy, hydrotherapy, and robotics- and virtual-reality-assisted rehabilitation. This is the heart of the recovery model, and the service most dependent on scarce therapist capacity, which is why technology-assisted rehabilitation and disciplined scheduling are central to extending each therapist’s reach.

  • Therapies: physio, OT, speech & feeding, neurodevelopmental, hydrotherapy, robotics & VR.

SERVICE 3 Paediatric Specialist Clinics

Consultant-led care

Outpatient and inpatient specialist clinics in neurology, orthopaedics, pulmonology, cardiology, developmental paediatrics, genetics, pain management and rehabilitation medicine provide the consultant expertise that guides each child’s recovery plan and feeds referrals across the network.

SERVICE 4 Child Development Centre

Assessment & early intervention

Diagnosis and management of autism, ADHD, learning disabilities and developmental delays, with behavioural therapy. Early, structured assessment and intervention improve lifelong outcomes and open a large, growing and under-served demand pool beyond the acute-recovery population.

SERVICE 5 Mental Health & Family Wellness

Psychological support

Child psychology, psychiatric services, family counselling, bereavement support and trauma recovery recognise that a medically fragile child’s recovery depends on the whole family’s wellbeing. This service deepens the family-centred model and supports both children and their caregivers through difficult journeys.

SERVICE 6 Home Healthcare Division

Hospital-to-home continuity

Following discharge, home nursing, therapy visits, remote patient monitoring, medical-equipment installation and telehealth consultations extend HarmonyBridge care into the home. This division is both a recurring-revenue stream and the clinical bridge that makes “hospital to home” real, sustaining recovery and reducing readmissions.

SERVICE 7 Parent Academy

Caregiver training

Structured training equips parents and caregivers in feeding, medication, physiotherapy techniques, emergency response, home care, nutrition and infection prevention. Empowering families to continue skilled care at home is central to outcomes, to reducing readmissions, and to the model’s social impact, and it generates training revenue.

SERVICE 8 Children’s Wellness Programme

Prevention & monitoring

Vaccinations, growth monitoring, nutrition, development screening and chronic-disease management extend HarmonyBridge into preventive paediatric care, broadening the patient base, supporting corporate and community wellness contracts, and reinforcing the brand’s role across the whole of childhood health.
